Is it the formresults.php page that displays blank?
I see a problme with the following code on that page:

iof your still having problems, try adding the following at line 1 of the page that is showing blank to turn error reporting:

error_reporting(E_ALL);
ini_set('display_errors','on');
?>
if Dreamweaver is showing errors about permissions on the server, try deleting the webassist directory from the server, then uploading it again.
