SQL Syntax Error

I am getting the following error when trying to use the xxx_Insert.php page:
"You have an error in your SQL syntax; check the manual that corresponds to your MySQL server version for the right syntax to use near 'update FROM member_info WHERE item = -1 OR ( -1= -1 AND item= 4)' at line 1"

A somewhat similar error occurs when I use the Search.php page.

A check of the Server Behaviours for Insert & Update indicate that the data types are all correct.

The page may be tested at: member_info_Insert.php

The database tables are attached as are the Pages generated by DataAssist.
