Ok, so if you use the edit button with a pencil next to it to create the email, there should be a lightning bolt icon in the bottom right corner. If you click on that, a Select Data Bindings panel should open. there should be an entry for your form in the fields box. Expand that and the different form elements should be listed. Select one of those to display the value in the email.

If you selected the edit button with the Dw icon next to it, you will have to add bindings through the bindings panel that Dreamweaver has. In the bindings panel, there should be an entry for your form and you can expand it to show the form fields. To add a display to the page, drag and drop the form field you want displayed to the correct position on the page. Then save the page and you should be taken back to the Universal Email Create Email Message dialog.
