I find that different forms need slightly different 'tweaks' to show the way you want them to.
The stylesheet for your form is your CSSFormBuilder/FD_Sharp_Default.css file. There may be a quicker and more obvious solution, but for me, what seems to work for your page is to change three bits of the stylesheet. [The changed bits are in blue]:
