You probably have to rename your configuration folder, please browse to:

C:\Users\*USERNAME*\AppData\Roaming\Adobe\Dreamweaver CS4\en_US

and rename your configuration folder, then open the extension manager and uninstall the extension and then try to install it again. That should resolve the error you are getting.
