error message after insert pg upload

Everything seems to be uploading fine to my database, but once I press, insert, it shows:

I've looked at this page, 148/ but cannot find any spaces. I've found some, taken them out, but I still get this message.

This is local on my computer. I haven't tried uploading it live, but I've changed the permissions on the folder.

I've attached the insert file. I get the same thing on the update page, once I added the file upload.

I didn't have this problem on the other section that I added the file upload to, but I added a drop down section for the categories.

I've done this a couple of times on the same one and continue with the same error messages, so there is no formatting whatsoever.
