User Registration Page not working correctly

Hi Everyone,

I need some help.

I used the User Registration Solution Pack to created a User Registration Page for my web site and connected it to my MYSQL. That works great. I was able to register myself and had a couple of clients access the page and were able to register and verify their sign up.

Once I got the page to work, I wanted to delete some lines of the original form.
So I deleted Phone Number, Fax, Address, Address Line 2 from the form using the instructions given to in this forum to another users that wanted to do the same thing.

The problem now is the forum does not work. When a visitor tries to sign-up the form keeps asking for a password. I did not alter this information.

I would appreciate any help in this matter. Please keep in mind I am just a beginner with CSS when providing detailed instructions on how to fix this problem.
