login page with optional register as private or company

I have generated a login page with Security Assist. But the login page I wanted is not the same as the automatically generated one. My login page should have the following optional code in addition to the code generated by Security Assist. This optional is in case the customer is not registered.

<h2> Register new customer</h2>
<form name="loginForm" method="post" action="">
<ul class="clean">
<li>
<input type="radio" class="customer_private" id="customer_private"
name="customer_private" >
<label for="customer_private">Private Customer</label>
</li>
<li>
<input type="radio" class="customer_private" id="customer_private"
name="customer_private" >
<label for="customer_private">Private Customer</label>
</li>
</ul>
<input class="submit" value="Registrera" type="submit">
</form>

Now I have a user_registration.php generated by Security Assist.
I can use that to make
user_registration_private.php and
user_registration_company.php.

My question is what code do I need to add so that a customer who is private is directed to user_registration_private.php and a customer who chooses company is directed to user_registration_company.php.
