Activation problem

Hello,
Operating system is Windows7

I am getting an error while trying to activate the FormBuilder extension.
While executing onLoad in wafb_serial.htm, the following javascript error occured:

At line 219 of file "C:\Program Files\Adobe\Adobe Dreamweaver CS4\Configuration\Shared\WebAssist\FormBuilder\Library\Shared_Ser_Mactivation.js": document.formLayer.document has no properties.

I disabled the Firewall, uninstalled the antivirus and I looked at some of the threads, even clean Dreamweaver's cache and delete the dat file, but still no joy,

I probably will have the same problem in activating every WebAssist extension?

Please!!, does anyone have any ideas how to fix this.

Assistance will be greatly appreciated.
