Issues with modifying exisiting form

Dear Friend,

I was able to successfully modify my existing form (removing old Spry code) and adding one more field — the security question. The CSS held and the form renders nicely on the page, but a few concerns:

1. In the Form Builder interface, I directed only two fields to be required (the browser preview was accurate), but the final form shows all fields required.

2. On the live page, none of the fields validate including the security question. Hitting the 'Submit' button without filling in any fields takes you right to the 'thank you' page. Seconds later, I receive the results (or blank form in this case) without trouble.

For the form action, I have a Pearl script processing the form (which has been working fine), and have removed all required fields from that, thinking it may conflict. Maybe this script is the problem?

I've attached a zip with the complete page and associated files. If you could take a peek at it and see if there is anything I'm overlooking, I would greatly appreciate it. Thanks in advance for your time and assistance!
