Validation Not Working

Hi, I am working in dreamweaver CS3. That's where I built the form. I am adding Spry Validation to the fields, setting up the preferences and when I upload its not working. Yes, I have also uploaded the Spry files to the server. Can you suggest what I am doing wrong?

The form is at:
schoolreg_index.htm

I presently have validate on question 1. I had it on every question but removed it when the error messages were just appearing.

Please advise.
