Hi Peter,

Are you sure that you are clicking the right button for Inserting a File Field?

To make sure, try getting it another way:

In the Dreamweaver menu, choose: Insert > Form > File Field

The pop-up window looks like the attached image, and does usually have all the correct options as described in the guide.

If it still doesn't, in the Dreamweaver menu, go to: Edit > Preferences > Select 'Accessibility' > and make sure that the 'Form Objects' check box is checked. (See second image).
