Menu element gives error after CSS Form applied to existing non CSS Form

The dropdown (menu) behavior as imported from a working Data Assist generated from does not seem to be working correctly.

Steps as follows:-
1) Initial form created with Data Assist. Each drop down Element on the Data Assist Form has one static item and a dynamic lookup to a record set.
2) Apply the CSSFormBuilder wizard to the form

When testing the page I get the following error:
====================================================================
Parse error: syntax error, unexpected T_ENCAPSED_AND_WHITESPACE, expecting T_STRING or T_VARIABLE or T_NUM_STRING in C:\Domains\**********.co.za\wwwroot\reg\em_participant_Insert.php on line 328
====================================================================
I then, through Dreamweaver(CS3), remove the dynamic list setting by setting the “Options from recordset” to none, applying this setting, reopen the dynamic list setting for the menu element, selecting the appropriate setting for the lookup recordset and applying the change.

Form is now ok.

Have I missed something in the steps for applying the CSS Form to an existing form or am I doing something wrong.

Full explenation and code generated in the attachment.
