Email Password for Security Assist v.1.18 not working for me

Guys,

I am trying to email a forgotten password.
My server does support mail () for php.
I have other sites on the same server where universal email works really well.
But the security assist function to email a forgotten password just won't work for me.
Please see the attached file.

I have tried a few things:

Left the server field blank and the from address blank - as per other posts.
It is still not working.
Is there something I have missed to setup this up?
Should I use either of these server addresses as an alternative to the smtp

Mail server IP address 213.171.216.66 (Cluster 1)
Host name for MX record smtpin1.livemail.co.uk
