Can't insert Form Builder data into record using DataAssist insert record

Using:
Latest WA extension version
DW CS3
WinXP SP3
PHP
mySQL

I'm posting this in the form builder forum since this is happening with a Form Builder form. (Have not had this problem with DataAssist pre CSS Form Builder.

I've created a custom form using CSS Form Builder. Using Data Assist's insert single record behavior, it does not insert the data into the record. A new record is created and the fields with default data are populated but all of the fields expecting form data are blank.

I have made sure that all CSS FB and DataAssist files are uploaded.

I then removed the Data Assist behavior and used the DW CS3's own insert record behavior. (same form, same page). Using DW's behavior works.
