discount and coupons not working

I setup coupons and discounts in my dev environment and they are working fine however when I modify my production pages nothing shows on the confirm page. I have ca coupon code for entering in the code and get 10% off, and I have another if you order $75 or more you get free shipping. Can you have a look at the code and tell me what's going on?

For the cart since it was not working I commented out the coupon form.
