Cannot locate hidden fields in registration form

Hello. When I set up my registration page for Security Assist, I chose to make the email address the User ID and I wanted to pass that variable in and not allow the user to change it. To accomplish this, I set it to be a hidden field in the setup wizard. My plan was simply to code the passed variable into the hidden field on the page after the wizard created the form. Unfortunately, I do not see the hidden field anywhere on the page that was generated. I see a label for it, but there is no field. Additionally, the primary key for my user database is set to auto increment, so I set that as a hidden field, as well and planned to leave it blank. For that one, I see nothing at all - no label or hidden field. Interestingly, I also set a third item to be a hidden field and that one showed up correctly on the generated form, but not the other two. Any idea what I have done wrong and/or how I could locate these missing hidden fields? I have attached the signup page that I was working on. Thank you.
