Unless you can reproduce the problem it will be just about impossible to determine what the cause may be.

Find out from the customer what products they where ordering and try reproducing the issue using those products in your cart. are there any calculations or discounts set up in your cart object?

Once you can reproduce the problem, add the full request binding to the checkout failure page to see what information is being sent to PayPal, this will help determine where the problem is.
