Data Assist Form with File Upload has stopped working

I have a form to add a news article and photo to a website. It has been working well for some time but has all of a sudden stopped working.

I don't get any errors on either insert or update however the file does not upload.

I have gone back in and have opened the wizards for both the insert and upload and have saved again to see if any files needed updating, however, this hasn't helped.

I am getting an error in the developer view though
