checkout page no working

I have used eCart with an Application form which I am testing locally but can not get the cart working. Im not sure if I have settings wrong or it is because I am testing it locally and not on a remote server. The application form directs to the cart page OK and the cart page displays the correct details. But when I click submit from the cart page it directs to the failed page. I have built a similar system to this before where it bypasses the checkout page with Shipping and Billing details and directs straight to PayPal which is what I want here. I have tried using live API credentials and Sandbox API Credentials but with no success.

Ive attached all relevant pages in the hope you can make some sense of where I am going wrong.
