Insert, update give syntax error when form is submitted

Please attach the actual php page so I can open it with Dreamweaver to see.

Try making sure your form elements and columns in your database don't have any special characters or spaces. I see you have a column with a "#" in the name and that could be causing issues. It also looks like a column may have a space in the name.
