Multi Record Input not inserting into database

It looks like you may have had your submit button inside the repeated selection at one point and then moved it out. Since it was inside it was renamed and now it is out of sync... just update the button from:

<input type="submit" name="insert_features_<?php echo $RepeatSelectionCounter_1; ?>" id="insert_features_<?php echo $RepeatSelectionCounter_1; ?>" value="Submit">

to:
<input type="submit" name="insert_features" id="insert_features" value="Submit">

Also, in the future, please upload the actual file instead of copy/paste its contents when you post to the forum. It is much easier for me to read if I can just open the file in Dreamweaver.

and that should get it working.
