adding file attachment

Hi,

Can you let me know why I cant get CV file to attach to this page healthcare-agency-jobs-register.php

I followed instructions to attach Submitted File in the Attachment menu, and it recognises the form file field CV, but doesnt attach/send. Emails come thru fin but no attachment. I did try to do file upload on this page and have the form go to registration-sent.php where i was going to trigger the email but a) the file didnt upload, and b) I wasnt sure what trigger to use on the receiving page - "on page load"?
