MySQLI User auth login user panel not displaying properly -missing store values panel.

FInd screen shot attached - I am using DW CC on a mac Yosemite ver 10.10.4
I used DMXZone extension mgr to install your extensions. DataBridge seems to work ok, but MySQLI is not. Any suggestions ?

DataBridge security assist keeps crashing when the OK button is clicked to create the pages. Nee help with this problem too. Thanks

Update: After uninstalling all my DW installations(2014 and 2015) using the Adobe CC cleaner tool and reinstalling only DW 2015 i was able to install the webassist extensions using the DMXZone installer. They seem to be working now. Thanks anyway.
