action buttons at bottom of cart (Continue Shopping, Clear Cart, Update, Checkout) have disappeared

I used display manager to add items to several cart columns and now all the action buttons at the bottom of the cart (Continue Shopping, Clear Cart, Update, Checkout) have disappeared, also the special instructions field i recently added. I looked at fix we did before on this but that fix does not seem to apply to the current file (see attached).

May have just figured this out: the display manager had "email friendly cart" chosen, which I guess was from the last time I edited the blank_2.php template page for the custom email receipt (didn't notice that at first, would've thought it would keep the info for the cart_static.php which I was editing).

Did the above and now it's really screwed up.Then tried to restore from our backup from earlier today. It now looks like something we had ages ago before I made many many changes. Am really at a loss...
