MySQLi Server Behaviors Extension - setup database connection failed

I have Dreamweaver cc2014 and successfully activated my new MySQLi Server Behaviors Extension and completed the popup WebAssist site, page selection and test server. I can not click on the +/- symbol in the database window to add a connection. I do not see Database listed under Window in the menu bar. I have a PHP page loaded and a sample database with one table created to connect to using the extension. I am using MySQL Workbench to setup the test dB and table. Can't proceed to use the extension....have read every tutorial and searched online to find a solution. What are the next steps?
