JavaScript Error - does not complete the checkout build

I have eCart 6.1.0, Dreamweaver CS4, and have built a cart with 19 columns and placed the cart display on the same .php page as the input and it works perfectly. Now I attempt to use the checkout wizard and get the following error.

While executing timer in WA_SiteTools_ProgressV2.htm, the following JavaScript error(s) occurred:

At line 415 of file "C:\Program Files (x86)\Adobe\Adobe Dreamweaver CS4\Configuration\Floaters\WA_ProgressV2\WA_ProcessingObject.js":
ConfirmPage is not defined

Needless to say when I click on checkout on my cart on my testing server it gives me a checkout.php page which is not properly formatted and it is incomplete. There is no where I can go from here.

I've attached a Word.doc that shows the error and the checkout.php page.

I have cleared the Dreamweaver cache and reset my views (seemingly common fixes) with no help.
