It's working now. I removed all of the email code and the include pages and then did the Create Email Message again.
Not sure what i did the first time.
Another problem - I set the email to send to two addresses but only sends to the first listed? // build up recipients array

Don't know if this has anything to do with it, but I have an array in one text field on the form so this code i added to the page which also added it to the index_WAUE_HTML_WA_Universal_Email_1.php
