Tried creating a File Upload Form. Why is it creating a second name and email field? (DB 1.2.1)

I used the wizard: WebAssist - Forms - Insert New Form. CSS Form Builder opens. I chose "Form Mail w Upload" from Choose YOur Form Fields. From the Preview I can see it already has an extra name field and an email field.

I proceeded to Form Function tab.In the FROM field I clicked the lightning bolt and selected the first e-mail address under FormMailUpload_NewFromBlank_Default. In the TO field, I just entered the client's email address. SUBJECT, I just filled in "Request Quote." I Unchecked the Save Uploaded Files in the lower area.

I haven't heard if the client received a test e-mail, but I'm wondering what the second NAME field and EMAIL 2 fields are about, and whether I've gone about setting up correctly.

I just want visitors to enter their contacts and comments and upload a file to be attached to an email that the client receives.
