In the future, please don't copy the php code to a word file, instead attach the original php file itself. It's more difficulty read php code in a word document than in DW.

text area elements do not use a value attribute. the format for a text area:

your text areas are using a value tribute rather than having the value between the opening and closing textarea tags:
