renaming files on upload

Was wondering if it would be possible to add First_Name and Last_Name from form fields to the upload "Params" on an insert page. Attempting to identify each image with the person filling out the form. I don't know php code that well, but it appears this would be the place to do this. I tried some simple configurations which added that text to the name of image file (and placed in database table), but not data coming from the fields - even attempted "echo" command. But again I don't know the code that well.

Here is the code from the page for the first image upload with just the FileName being used:
