Setting up second form

I'm having a major issue with setting up a form page i created. I have one form that is working and I wanted to create another from in the same directory.

I don't seem to be doing this right, because thinking is working not even the button which maybe is the issue but I cant figure it out.

Any help would be GREATLY appreciated.
