Can Form Builder be used to recreate this example....

I spent some time with trying to add a form to a test page and make it work. No luck. I followed step by step off the video on YouTube and It didn't submit anything. Seems like the builder does a lot more than was not documented. Do you have to have a Database and A Recordset to create a Form with Form Builder? I have not hand any luck with any of the WA extensions within Data Bridge yet. I simply can't get anything to work properly. It must me me, I can't seem to follow the tutorials and the videos seem outdated.

It would be nice to see actual working demos or have test files to work with.
