How do I do a bill pay where client puts in amount?

In using eCart6.08 I want to create a form where the client inputs the amount to pay, invoice number, and billing information to send to payment gateway authorize.net. I don't want the form to send them to a cart page or get cart information, just ask for confirm then send information and then return them to checkout success page as now.
