MySQLi Insert Record combined with File Upload - File name not saving to database?

Where I'm doing an UPDATE form, what should the default value of the img file field be? Also, should the column binding be the same as I'm using in the INSERT form?

I've got the INSERT form working following your advice above but when I create an UPDATE form it's deleting the file name saved in database, which suggests that it's submitting but it's submitting a blank value.
