validation tool tips not showing on ALL required areas in form

Hi I'm working on a form where not everything is required.
But the validation tool tips only come up on the upper half of the form and Not the required items at the end.
Where am I going wrong?

Also I need to have the credit card validate for only Visa & Mastercard.... I don't know if I did it correctly.

Please let me know how I can fix this.
