DFP/DA only displaying duplicate filenames on DA Detail pages

Please can I have advice on how to solve this urgently?

Customers are placing orders (via a DA Insert page) which includes an image uploaded using DFP2. DA Detail pages show the customer the final product before they decide to pay.

My BIG problem is that the only images which display correctly are duplicate file uploads i.e. ones where DFP has renamed the new file to [FileName]_[Increment]. NONE of the non-duplicate images display at all!

I am going completely nuts over this - everything seems to works perfectly for the incrementally renamed image files - but NOTHING displays for a non-duplicate image uploaded (i.e. I can't launch commercially until this is solved as customers obviously have their own filenames). The original files themselves are uploading to the server fine!

I can only rename files in DFP2 if the filename exists already else I'd just rename everything!

Please can you tell me when I have to do to correct this frustrating situation ASAP - I really am frantic. What am I doing wrong?!
