Help with update page - not redirecting to page

I have a page here with a form, which works, which is just a basic form which is inserting the contents into a database table and emailing the contents.

http://www.safariawards.com/nominate/

And I have pages like this one with details of a particular lodge, e.g.:

http://www.safariawards.com/botswana/africanhorseback/

I have been creating new page which combines the two, i.e. with the lodge details, and a nomination form.

If you go to this page:

http://www.safariawards.com/finalists/

Any links will take you to the new page, which is using a URL rewrite, e.g.:

http://www.safariawards.com/lodges/africanhorsebacksafaris/

Which is using a URL rewrite to display www.safariawards.com/lodges/index?GSG_URL=africanhorsebacksafaris

The form is working in that it is writing the contents to the database correctly, but it is drawing a blank when submitted rather than redirecting to the specified redirect page. The email isn't being sent either, but hopefully caused by the same issue.

I'm hoping its nothing too complicated, and just something I have missed, or have in the wrong order at the top of the page.

I have attached a copy of the page, and would be grateful if anyone could have a look to see if they can spot what's causing the issue.
