Hi,
I’m not sure what type of input form I need to build to achieve the following; can you please advise whether either the standard Insert or Update forms produced by the Data Assist Wizard will achieve this?

I have 16 columns in the database. 12 of these are to be updated by a separate update form which is already in place.

The other 4 I need members to be able to insert/update via the website to achieve the following:

Example

Form fields to be inserted/updated:

Name of Supervisor (text)
Type of Supervision (menu)
Date (datepicker)
Hours (numeric)

What I need to happen is for the entries to be added to, not replace, any existing entries.

e.g.

If there are existing entries like this (or no entries)

Name of Supervisor Type of Supervision Date Hours
Jane Bloggs Professional 22/05/2013 2

And they next have a session with John Doe of Peer Supervision on 03/07/2013 for 4 hours, I need them to be able to insert the John doe details and have the database columns and results page show this:

Name of Supervisor Type of Supervision Date Hours Total
Jane Bloggs Professional 22/05/2013 2
John Doe Peer 03/07/2013 4 6
