Are you redirected to the Index.php page or does the page just refresh? if the page just refreshes, the problem is the validation
one issue i see is that you have 4 instances of the Server Validation server behavior.
there should be only one instance of Server Validation. you can add multiple validations to the one server validation instance.
If the page redirects to the index page, the problem is the settings used for Universal Email.
Double check the From Address, make use it is a valid email address for your domain.
also, double check the Email Server Settings, depending on the host, you may need to enter an SMTP Server Address, or you may need to set the option in the from Settings to include the from address as an Extra Parameter.
