Registration Form/forgotten password pages not working

Errors have crept in to my pages, which I probably have to take full responsibility for. I tried to introduce a second login system to be located in a different folder but I suspect certain pages for my original system were effected. I realised after doing so that it was a big mistake in what I had done and tried to recover to original files with Time Machine (Mac). Subsequently parts of my Registration/login system are not working.

Problems are as follows:

a) The registration form does not submit the details to the Database Table
b) The forgotten password form does not work.

Please someone help.. (probably Jason). I can send ftp details which I expect will be required. I realise that this is probably my errors that are the cause so will be happy to submit a paid ticket if required.
