0 error ... Anyone else getting this?

I have reinstalled all extentions in the order listed but still seem to get this error when going to webassist - creat email - (setup fields) click Finish - get 0 ERROR.

I am currently doing this tutorial data_bridge_create_double_opt_in_reg.php and when I get to the first part that is adding the email to the registration page, I set all the setting per the tutorial and click finish and the error pops up. It still creates the Blank.php email page and when I press the ok on the error I can save the registration page. I'd like to know if everything that is supposed to happen before the error occurs happens so I am attaching the registration file so you guys can see weather or not there is an actual issue or not. Thanks!
