DataAssist Create Pages

Dear Jason, I am creating some user page with DataAssist Create page. See the attached message containing two screenshots.

The top screenshot gives me a message, but prior to that, I was unable to bind the 'to' part of the email settings, because I cant't see the form in the pop-up screen.

How do I bind and get it to send an email with the registration details to the person who is registering?
