the #1 rule when working with forms is:

All form processing must occur on the page specified by the action attribute of the form tag.

in your page, the form tag:
<form action="serviceReqConfirm.php" method="post" name="WADAInsertForm" id="WADAInsertForm">


has the action pointing to another page:
serviceReqConfirm.php

but you have the universal email behavior on the same page as the form, because the action points to another page, UE will never be triggered.


change the action attribute to be blank so it will post to itself. In universal Email, set the go to page to redirect to the serviceReqConfirm.php after the email is sent.
