Dreamweaver CS4 crashing after installinf Form Builder

I just purchased the Form Builder and after I installed the extension and set up my first form Dreamweaver crashed on me. I reopened it and it continued to crash every time I attempted to do anything including changing sites or connecting to the server. I deleted the form files and reset the site up I was working on to no avail. Finally I uninstalled the extension and everything is working fine. Anyone else have this problem?

I'm running a macbook 2.4 ghz/ core 2 duo processor and osx 10.5.8 along with DW CS4.

Any help would be appreciated.
