forgot password error messages

Hello~

I'm having some trouble with the 'forgot password' page generated by security assist. I've been getting a series of errors that I have been able to troubleshoot, but I can't find what the problem is with why I'm receiving these:

Notice: Undefined index: WA_MailObject_Error in /webassist/security_assist/mail_php.php on line 262

Warning: Cannot modify header information - headers already sent by (output started at /webassist/security_assist/mail_php.php:262) in /webassist/security_assist/helper_php.php on line 283

I've checked for the usual culprits - spaces, extra lines, etc....

Thanks~
