Need to add a spry validated checkbox to a form

Hi, How do I add a spry validated checkbox to a form that was created by security assist?

I have tried to recreate the form, but I can't see how I can get a checkbox added if it doesn't have an equivalent database field to insert to. All I need the check box to do is validate on submit that the user has agreed to and checked the terms and conditions. If they don't tick the checkbox then the form doesn't submit and it returns an error.

What do I use for this? I have tried the built in DW validate spry, but that didn't seem to do much!
