in the upload behavior, you have set the file name to use the Server File Name binding.
this is circular logic.
The Server File Name variable is created once the file is uploaded, and renamed if necessary based on the If File Exists Settings. It is created by the upload behavior, and does not exist until the upload is complete. this binding cannot be used as a setting in the upload behavior.
in the upload behavior, set the File Name the default "[FileName]" instead.
the server file name binding should be used in the insert record behavior, not the upload behavior.
