CSS Form Builder Installation Error

Hello Ray,
When I purchased CSS Form Builder, I installed it without uninstalling the Validation Tool Kit. Later, I uninstalled the Validation Tool kit and the CSS Form Builder. I then downloaded the CSS form builder and installed it. When I tried to Insert form using CSS form builder, I received the following Error:

While execting onLoad in WAFB_AddForm.htm, the following Javascript error(s) occurred:
In file "WAFB_AddForm": initializeUI is not defined.

I later uninstalled the CSS Form Builder and reboot the computer and re-installed the CSS Form Builder. I then tried to enter the CSS form builder by choosing Insert > WebAssist >CSS form builder > Insert Form, I received the same error as above.
