Authentication produces Blank Page

Hi folks,
I have created a folder with login/registration/etc. And I added to one page
dealers.php
Access Pages Manager, requiring a login for this page. It should redirect to login.php in the same folder if access is denied, but what is happening is I am getting a blank white page.

Here's the top code for the page:

The references pages are uploaded.

Any ideas? My goal is to, once the user is logged in, to come to this form page, and auto fill the dealer's ID and address. I'm using DW CS 5.5 for this section of the project.
